Prescription Policy
On our website, certain items can only be purchased with a prescription, and these items are clearly indicated as such. It is our policy to confirm you have the necessary prescription and information about the equipment you are purchasing.
Prescriptions and relevant paperwork can be verified in a few ways:
- Complete the following form and upload the prescription
- Email the prescription to info@agilitymedical.com.au
- Provide us with the correct phone number when placing the order – The billing address must be in your name – Our team member will reach out and confirm
Handwritten prescriptions using a standard pad will be accepted. However, your prescription must include the following information:
- Name of physician
- Physician contact information
- Physician signature
- Information about the needed equipment
- Pressure setting information for CPAP / Bi-level machine
For patients who are simply replacing or buying a second machine, evidence of previously diagnosed or treated Sleep Apnea is required.

- select the file you just made.
- complete the name and email fields.
- click submit and wait for your file to be transmitted to our system.
